As an agent of the U. S. Department of State, the Probate Court will accept applications for passports. Applications are processed Monday through Friday from 9:00 A.M. to 3:30 P.M. and in the months of July -August Monday through Thursday from 9:00 A.M. to 3:30 P.M. Fridays 9:00 A.M. to 1:00 P.M. They are to be mailed out the day the application is submitted. All forms are available in the Probate Office and may also be printed out on line from Passport Forms
Special Requirements For Persons Under 14 Years of Age:
Child must Appear
Both parents or the child's legal guardian(s) must appear and present the following:
Evidence of child's U.S. citizenship,
2 passport photos
Evidence of child's relationship to parents or guardian(s), and
Parental identification
If only one parent appears you must also submit one of the following:
Second parent's notarized written statement consenting to passport issuance for the child,
Primary evidence of sole authority to apply, or
A written statement (made under penalty of perjury) explaining the second parent's unavailability.
Initial Passport Issuance:
(Persons 15-17 years of age must appear with at least one parent)
Proof of U.S citizenship
Proof of identity
Two passport photographs

The passport fee must be paid by check. The execution fee may be paid by separate check or cash.
Passport Renewals:
Most renewals are handled by the applicant directly through the U. S. mail with the exception of passport issued to persons who were under 16 at the time the old passport was issued, these renewals require a full application.
