When you visit or call Human Services, we can give you information on health services, housing options, adult day care, legal aid, transportation, mental health counseling, financial assistance, volunteer opportunities, classes and activities, Meals on Wheels and much more. How do I qualify for ConnPace (Connecticut Pharmaceutical Program)? If you have been living in Connecticut for six months, are 65 years of age or older, or disabled over 18, and your adjusted gross income for the previous year plus Social Security (minus Part B premiums) is less than, for a single person: $23,700, married: $31,900, and you do not have an insurance plan that pays for all or a portion of prescriptions. If you have an insurance plan with a maximum benefit, you may submit an application. How do I apply for Fuel Assistance? Call the Department of Human Services and schedule an appointment. The State Fuel Program runs from October - March. We will ask you to bring in proof on income, assets, age and disability (if applicable). If you are over the income limit for the State Fuel Assistance Program we will have you apply for the local program. How do I apply for subsidized housing in New Canaan? Call the Department of Human Services at 203-594-3076 or call Phoenix Management at 203-324-2154 x16. Subsidized housing includes Canaan Parish which has 60 units, Millport Apartments which has 18 units, and Mill Apartments which has 16 units. Final decisions rest with Phoenix Management per the appropriate subsidized housing regulations. Are there free legal services available if I cannot afford an attorney? Yes, you can call Legal Services at 800-453-3374. Could you describe Lifeline and how I would qualify? Lifeline is a small waterproof button that you wear around your neck or around your wrist. When you need help, any time of day or night, just press the button to send a call for help. The button can work even when you are in your yard or basement. You can get Lifeline by calling 203-853-3116 and a caseworker will follow up. Do you take donations of furniture? We do take donations of furniture only if we have a client who needs what you are donating. Do you have any free furniture donations? We have free furniture donations for the clients we are working with. How do I qualify for Meals on Wheels? Any New Canaan person who is unable to shop for, or prepare, adequate, nutritious meals, who is recovering from illness or recent surgery, who has a temporary or permanent disability, or whose family is not available to help is eligible for Meals on Wheels. The Getabout Van provides transportation to elderly and handicapped New Canaan citizens and adult New Canaan residents who cannot drive themselves due to no car, poor eyesight or an injury or illness. The Getabout has just started taking New Canaan residents to Norwalk. They ask for a voluntary contribution of $3.00. To reach Getabout, call 203-972-7433. Town-To-Town provides transportation for out of town appointments only. This is a regional door-to-door transportation service for people who wish to travel between towns in the Southwestern Connecticut Region. The cost is $4.00 for a one-way trip across one town line. To reach them call 203-299-5180. The Red Cross will transport New Canaan residents to medical appointments only. There is no charge and you need to call 203-966-1663 to reserve a ride. The Municipal Agent serves as a liaison with Health and Human Services and other resources and programs serving the elderly. She is available to all seniors in the community who may have questions about services. We have programs that provide meals, gifts and support during the holiday season to the families we are working with. I am a homebound senior and want to know if a counselor can do a home visit?
